That one question settles most of the diaper or underpad debate. An adult diaper is worn on the body. An underpad lies flat on the bed, sofa or seat. They do different jobs. Sometimes you need one. Sometimes you need both.

When underpads alone may be enough

Some people find diapers uncomfortable in bed. A pad under them may feel lighter. Ask the nurse what suits your patient before you switch.

When an adult diaper makes more sense

• The person sits in a chair, walks with help or goes out.
• Leaks are heavy or frequent.
• Nobody can check the bed for long stretches, for example during work hours.
• The person wants to stay dressed and active.

When to use both

At night

Night time is the classic case. The diaper does the main job. The underpad catches what escapes, so you change a pad instead of a full set of sheets at 2 am.

On trips and family visits

A diaper keeps the person covered on the move. A pad on the bed where you stay protects your host’s mattress and saves everyone an awkward morning.

After surgery

The first nights home after an operation can be messy. Follow the hospital’s advice on care. A pad on the bed or chair is cheap insurance for the mattress.

What it costs: a simple week

Here is an example for someone who needs four changes a day. Diaper prices vary a lot. Packets of 10 on Jumia ranged from about KSh 975 to KSh 3,600 in October 2026. We used about KSh 130 a diaper, the price of one packet of 10 we saw at Pharmily.

Cheaper is not always better. Pick what keeps the person clean, dry and comfortable. Then find the best price for that plan.

What about washable underpads?

Some shops in Kenya also sell washable, reusable underpads. One Nairobi seller listed them at KSh 3,500 in October 2026. They suit some homes, but they need washing, drying and a spare while one is on the line.

Adult diapers

Diapers come in sizes by waist, from medium to XXL. A diaper that is too big leaks at the legs. One that is too tight rubs. Measure the waist and check the size chart on the packet.

Underpads

Underpads come in one size per product. A 60 x 90 cm pad covers the area under the hips on most beds. Lay it flat with no folds and use the adhesive tape to help it stay in place.

Questions to ask before you buy

1. Does the person stay in bed most of the day?
2. Are leaks light, moderate or heavy?
3. Can someone check and change pads during the day and at night?
4. Does the person want to sit up, walk or go out?
5. What has the nurse or doctor advised?

If leaking is new or has changed suddenly, see a doctor. It can have a cause that needs checking.

Talk about it with care

Many adults feel shy about leaks. Use simple, kind words. Say “pad” or “bed protector” if the word “diaper” upsets them. Let them choose where they can. Dignity matters as much as a dry bed.
